PIA is organizing Online Consent Issuance Mela on Friday, the 7th December 2018 at 10:30 am PIA Auditorium for the members who want to get the consent kindly bring the following documents.
- Aadhar No. For E- sign
- Identity proof(PAN card, Voter ID, Driving License
- Authorized signatory ( if applicable)
- SSI certificate
- Project report indicating Manufacturing process, Location, water source, Product, Raw materials, Water consumption, wastewater generation & control measures, Air pollution details, Solid waste details, Hazardous waste details, Number of employees.
- Process flow chart
- Land/premises details – Rent agreement/Sale deed/Lease deed
- Location map
- Copy of Partnership/ company deed
If applied for consent renewal
- Compliance to previous consent conditions
- Environmental Statement( Form 5 )
- Annual report under HWM rules (Form 4)
- Analysis report of effluent/ Stack emission
